Transforming Lives for Senior Veterans in Lee County

Reducing Hospital Readmissions

In Lee County, up to 15% of senior veterans are readmitted to hospitals within 30 days due to inadequate post-discharge care, costing our healthcare system an estimated $2.28 million annually. Your donation funds home-based healthcare and resource coordination, cutting readmissions by up to 50% and saving over $1 million while helping veterans recover with dignity at home.

Restoring Independence

Your support empowers senior veterans to live independently and thrive, providing tailored healthcare services like in-home care and mental health support to prevent hospital readmissions and promote overall well-being. Every dollar donated makes a tangible difference, whether it's funding essential medical equipment, covering critical expenses, or providing vital therapy.

Bridging Gaps and Enhancing Quality of Life

Your contribution helps build a stronger community for Lee County's veterans and their families by fostering collaboration among local organizations, eliminating duplicated efforts, and creating a unified support network. By bridging resource gaps and connecting veterans to essential services like transportation, housing assistance, your donation ensures seamless support and enhances their quality of life through partnerships with trusted organizations like Lee Health and Mission United.
